As pipers, we occasionally make mistakes, errors, design flaws etc.
We normally have our fail-safe - the checker ... but occasionally, something gets past the checker too.
What have been your experiences, personal or anecdotal, of errors that have escaped from the board and made it into the field?
(I know piper will want to admit these - so let's pretend that someone else did it!)
