The simple answer:
"The same level of detail (graphics, dimensions & notes) as any other piping or piping related item needing to be fabricated."

Trunnions on Iso’s would “normally” be depicted in a very simple graphic symbol. The main goal for an Iso drawing is to locate where this trunnion occurs on the piping spool. Details for the trunnion would either be cover in engineering/design support standard drawings, where it would be given a name …“TS-1” (for example) …the simple graphic symbol on the spool would have this TS-1 name and verbiage would indicate to go to the company standards drawings for a full detail of this trunnion.
Companies with no standard drawings would also use a simple graphic symbol for the trunnion, with verbiage to go to a separate detail (either on that Iso sheet or some detail drawing) where that trunnion is also fully detailed & documented. Trying to generate a detailed trunnion in an isometric style, proportional to the Iso drawing “scale” would be very hard to do (in a clear manor) and confusing.
